First Appointment It is in your best interest to arrive a few minutes before your appointment. There will be a short health questionnaire to fill out.
Illness & Health Concerns Please let us know of any health conditions, allergies, or other concerns you may have prior to treatment. In an effort to provide a healthy environment, we respectfully require that you reschedule your appointment if you are experiencing a cold, fever, flu, etc. This is in consideration of your health as well as the well-being of other clients and staff.
